Skilled Nursing (RN, LPN/LVN)
Skilled Nursing is performed by an RN or LPN with training and experience in providing care in the home. The nurse communicates frequently with your physician to update your plan of care. Services may include evaluation of patient needs, performance of skilled nursing procedures, education of patient, family members and caregivers on disease processes, self care techniques and prevention strategies, and coordination of patient care and services with your physician and other health care team members. Specialized services are offered in the areas of ostomy care, infusion services, and wound care.
Certified Nursing Assistant
Certified Nursing Assistance services are provided when applicable as long as you are receiving either skilled nursing care, physical, or speech therapy. The Certified Nursing Assistant services are delivered under the supervision of a licensed nurse or physical therapist.
Physical Therapy
Physical Therapist provide therapeutic exercises, home exercise programs, training in use of adaptive equipment and restorative treatment to help you return to your maximum level of independent function and capabilities.
Occupational Therapy
Occupational Therapist provide exercises and activities to help you become more self-sufficient in doing your normal daily activities in caring for yourself.
